If you are an adult with type 2 diabetes or are a caregiver to someone with type 2 diabetes and live or work in Rockland County, you can take part in our free workshop series, Living Well with Diabetes (Diabetes Self-Management Program)*.
The workshop series will help you understand diabetes, learn how to prevent complications, keep track of your blood sugar, eat healthy, be more physically active, learn relaxation techniques to cope with stress, and take charge of your health.
The workshop series meets for 2.5 hours, one day a week for six weeks. Our trained facilitators will help you gain the skills you need to better manage your type 2 diabetes and improve the quality of your life.
Along with the covered topics, group participation and sharing are encouraged. The mutual support and success participants get from the group process builds confidence in their ability to manage their health and maintain an active and fulfilling life.
Participants receive a copy of the companion book, Living a Healthy Life with Chronic Conditions, and an audio relaxation tape.
